Otherwise, provide a brief, general description of yourself (if this form is for you, an individual performer) or your group (if this form is for a group for which you are the contact) for inclusion in the program book and on the NEFFA web site. Please limit text to 10 words or fewer.

For a group, please list the names of all PERMANENT members. Don't forget to include yourself (the contact) if you are also a member of the performing group. DO NOT LIST TEMPORARY additions or deletions to your group for THIS YEAR!--please indicate these temporary changes on the EVENT APPLICATION (see question #4). This information is used for the program book listing, for the list of performers to receive tickets, and for preventing programming conflicts.
